Why Did My Dark Spots Get Darker After a Peel or Laser?

November 02, 2026 2026년 11월 2일 2 novembre 2026

In brief: Usually one of three things has happened. In the first week or so after a pigment laser, treated spots are meant to darken and flake off. If darkening appears two to six weeks later, it is most likely post-inflammatory hyperpigmentation (PIH): the controlled injury inflamed the skin, and melanocytes answered by making more pigment. If the darker areas are blotchy, symmetrical patches on the cheeks, forehead or upper lip, the treatment may have stirred up melasma. The first two are common and usually fade; the third needs a physician's plan.

Why a treatment meant to lighten can darken

Peels and lasers both work by injuring skin on purpose. A peel dissolves layers chemically; a pigment laser heats melanin until it fragments; resurfacing lasers vaporize or coagulate columns of tissue. Repair brings inflammation, and the same signals that drive healing, such as prostaglandins and cytokines, also tell melanocytes to produce more melanin and pass it to the surrounding keratinocytes. Skin with more active melanocytes, meaning medium to deep skin tones and skin with existing pigment problems, answers more strongly. Sun on freshly treated skin adds a second push.

The numbers show this is not rare. In 516 Korean patients whose solar lentigines (sun spots) were treated with a Q-switched 532 nm laser at five hospitals, 20.3% developed PIH. It was more frequent when the spots were reddish before treatment and when the face already had diffuse uneven pigmentation; age, sex, skin phototype and season made little difference in that group. In a small randomized study of the same laser, PIH appeared in 55.3% of treated lentigines in the group using only an emollient; the risk rose with redness still visible at two weeks and the odds were about eight times higher with outdoor activity between 1 and 5 p.m. Resurfacing behaves similarly: in a split-face trial of 25 Asian patients with acne scars, 24% developed mild PIH on the side treated with a fractional CO2 laser, against none on the side treated with a fractional picosecond laser.

Expected darkening, PIH or melasma?

What you see When it appears Most likely explanation What it usually means
Treated spots turn darker or greyish and form a thin, fine crust Within hours to the first 7 to 10 days after a pigment laser Fragmented pigment rising to the surface to be shed Expected; it flakes off on its own if left alone
A brown halo or patch in or around the treated area, after it had looked clearer About 2 to 6 weeks after the treatment PIH from the inflammation of healing Usually fades over weeks to months with sun protection and no new irritation
Blotchy, symmetrical brown patches on the cheeks, forehead or upper lip, darker than before Weeks to months, often after heat- or light-based treatment Melasma worsened or revealed by the treatment Relapses easily; treatment belongs to a physician
Flat pink to red marks that blanch when pressed Days to weeks Post-inflammatory redness from small blood vessels Not pigment, so brightening products will not change it

Melasma deserves a separate warning. A review of medical treatments for melasma notes that peels, lasers and light therapies have limited evidence in this condition and often cause PIH, particularly in darker skin, which is why medical therapy and strict photoprotection remain the mainstay. If you are unsure which you had to begin with, how melasma differs from sun spots sets out the clues.

What to do while it settles

  1. Leave the crusts alone. Picking extends the inflammation and pushes pigment deeper, which turns a short problem into a long one.
  2. Protect from light every day. Use a broad-spectrum sunscreen your healing skin tolerates, a tinted formula if you are prone to pigment, plus a hat, and keep midday hours outdoors to a minimum for the first six to eight weeks. Follow your provider's instructions on when sunscreen can start after the procedure.
  3. Calm before you brighten. While the skin is still pink, tight or stinging, keep to a gentle cleanser and a plain moisturizer. Healing skin holds water poorly and lets irritants in more easily, so an active added now irritates more and works less, and irritation is itself a trigger for PIH. Why skin is more sensitive after a peel explains this first phase.
  4. Ask before starting a pigment treatment. In the randomized study above, a prescription triple-combination cream started two weeks after the laser lowered PIH from 55.3% to 31.0% of lesions. That is a prescription decision for your physician, not a reason to stack over-the-counter acids on healing skin.
  5. Photograph and wait. Take photos in the same daylight every two weeks. In a study of a picosecond laser in skin types III and IV, the hyperpigmentation that occurred in 14% of patients had resolved within three months.
  6. Pause further treatments until the skin is even. A repeat peel or laser on top of active PIH adds inflammation to inflammation. When you do plan the next one, tell the provider what happened; preparing skin before a peel covers what changes for pigment-prone skin.

Hold off on scrubs, at-home peels, retinoids and strong vitamin C until the redness has gone, and avoid saunas, hot yoga and long hot showers in the first weeks, since heat sustains the inflammation.

When a physician needs to look

See a physician or dermatologist if the darkening is still there after three to six months, if it spreads beyond the treated area or forms symmetrical patches that suggest melasma, if white patches appear where pigment has been lost, or if the area blisters, weeps, crusts thickly, becomes increasingly painful or shows pus. A treated "spot" that returns quickly, or one that was never examined before it was lasered, must be checked by a physician, because a laser can hide a lesion that needed diagnosis. Prescription lightening creams and oral medications are medical decisions.

Where the research stops

Most of these studies come from Korea and Thailand and involve specific lasers on specific lesions, so the exact percentages will not transfer to a peel or another device. The prevention trial had 28 patients, the CO2 trial 25 and the picosecond study 22. Studies of how long PIH lasts after peels in North American skin of every tone are scarce, and the timeline in the table reflects clinical experience as much as trials. The direction is consistent, though: inflammation and light drive the darkening, and less of both lets it fade. If marks from acne rather than a treatment are the issue, why dark marks linger after a pimple covers the same biology from that side. Treat this article as education about how skin heals: it is not a diagnosis, and it is not a substitute for an examination by a physician or dermatologist.

Before the next treatment

Darkening after a peel or laser usually says more about the skin's readiness than about the device, and the next step is often to wait. Start with The Skin Read, the free questionnaire that records your skin type, pigment history and current routine. Then bring the result and your treatment photos to a complimentary 45-minute consultation, where "not yet, let it settle first" is a normal and useful answer.

Why Does Wearing a Mask Cause Breakouts?

November 02, 2026 2026년 11월 2일 2 novembre 2026

A mask traps heat and humidity, raises skin pH and sebum, and rubs the follicles it covers, so they plug more easily. Fresh masks, fewer hours and a simpler routine help.

Tretinoin vs Retinol: What Is the Difference for Sensitive Skin?

November 02, 2026 2026년 11월 2일 2 novembre 2026

Tretinoin is retinoic acid itself and a prescription drug; retinol must be converted in skin first, so it is weaker but gentler. Which suits sensitive skin, and who decides.

Should You Store Skincare in the Fridge?

1er novembre 2026

Most skincare does not need a fridge; a cool, dark cupboard does more. Refrigerate an opened pure vitamin C serum and anything labelled to refrigerate; cold can harm creams and oils.
